PThe following are documents and forms distributed by St. John’s Child Care Center. If you have any questions about these documents, please contact us. To view files, you will need Adobe Reader.

• Application Form (Fall & Mid-year)
  This application enrolls a child for a school year (September through May).

• Application Form (Summer)
  This application enrolls a child for the summer session (June through August).

• Parent Handbook
  Updated October 2023. The Parent Handbook contains all of the program policies and procedures. Please contact the Child Care staff for current rates at 612-827-1237.

• Health Care Summary
  All children need to have a current Health Care Summary on file.

• Prescription Medication Authorization/Administration
  Necessary for a child to receive medication while on-site.

• Immunization Record
  All children need to have a current Immunization Record on file.

• Permission Form
  This document permits a child to participate in designated activities.

• Sunscreen Permission Form
  This document permits a child to have sunscreen applied during outdoor activities.
