Application Form (Fall) 

This application enrolls a child for a school year (September through May).

Application Form (Summer) 

This application enrolls a child for the summer session (June through August).

Parent Handbook 

The Parent Handbook contains all of the program policies and procedures.

All program fees are billed according to this rate sheet.

Health Care Summary 

All children need to have a current Health Care Summary on file.

Prescription Medication Authorization/Administration 

Necessary for a child to receive medication while on-site.

Immunization Record  

All children need to have a current Immunization Record on file.

Permission Form 

This document permits a child to participate in designated activities.

Sunscreen Permission Form 

This document permits a child to have sunscreen applied during outdoor activities.
